Individual Plans
Basic Wellness Dental Plan - $80 per year.
-Save 20% on all dental procedures including cleanings, exams, x-rays, fillings, root canals, crowns, dentures and implants.
-Members must use our network of dental practices.
Premier Wellness Dental Plan - $350 per year or $30 per month.
-Two cleanings and exams plus necessary x-rays per patient per calendar year.
-Save 25% on all other dental procedures including additional x-rays, emergency exams, fillings, root canals, crowns, dentures and implants.
-Members must use our network of dental practices.
Family Plans
Premier Dental Plan - $550 per year or $46 per month for 2 members
-Two cleanings and exams plus necessary x-rays per patient per calendar year.
Save 25% on all other dental procedures including additional x-rays, emergency exams, fillings, root canals, crowns, dentures and implants.
-Members must use our network of dental practices.
- Each additional Member pays $100 per year per member.

Allenstown, Londonderry, Nashua, Raymond, Tilton and Weare.
Eligibility
Memberships are offered to individuals, families and families with children without dental insurance.
CHILDREN WHO ARE BENEFICIARIES OF ANY FEDERAL OR STATE HEALTH CARE PROGRAM ARE PROHIBITED FROM PARTICIPATING IN THE DENTAL WELLNESS PROGRAM.
